Sculptra is an FDA-approved biostimulatory injectable that works differently from traditional dermal fillers. Rather than adding immediate volume, Sculptra instead stimulates collagen production over time. The results appear more gradually than with HA fillers, but allow for more natural-looking improvements in facial volume and skin quality. How Does Sculptra Work?

Most fillers use hyaluronic acid to add volume to the injected area. Sculptra relies on a different material, poly-L-lactic acid (PLLA). PLLA is a biodegradable substance with a history of safe use in medicine spanning decades. Once injected, it acts as a scaffold that encourages the body to rebuild its own collagen. Over time, it helps address volume loss at its source rather than simply filling space.

What to Expect During a Sculptra Session

We begin with patient comfort. While not all patients will need any numbing, we have two options available. Topical numbing can be applied to reduce the pinch of the injection, and we can also mix lidocaine into the formula to reduce post-injection discomfort.

Even without any numbing, the injections are quick and typically cause minimal discomfort. The session then involves injecting Sculptra into the target treatment areas. Patients can treat a single area, or multiple areas depending on the volume loss they wish to address. You can expect about 15–30 minutes for the injections from start to finish. Once the treatment is complete, most patients can return to their daily routine.

Downtime & Recovery

There is minimal downtime after Sculptra injections. However, patients should avoid strenuous activities, like exercise, for 24–48 hours to help let swelling resolve. Mild swelling, redness, or bruising at injection sites is common but typically resolves within a few days. Patients are usually advised to massage the treated area for several days following each session, as directed by their provider. Our typical recommendation follows the 5-5-5 Rule, which means:

  • Massage the area for 5 minutes per session
  • Massage 5 times per day
  • Massage for 5 days post-treatment

Who is an ideal candidate for Sculptra?

Sculptra is well suited for patients experiencing overall facial volume loss due to aging, namely in the cheeks, temples, and lower face. While generally safe, we still fully consult with each patient and review their medical history to minimize any risks.

The treatment is generally open to patients 18 and older, with most patients being between 30 and 75 years old. Also, some key candidate qualifications include:

  • Not currently pregnant or breastfeeding
  • No active skin conditions
  • No history of scarring tendencies
  • Not having certain autoimmune conditions

Sculptra works by stimulating collagen production over time. Therefore, the results build gradually over several weeks to months rather than appearing immediately. Most patients notice initial improvement within 4–6 weeks after their first session. Results will continue developing for several months after completing their recommended treatment series.

Most treatment plans involve 2–3 sessions spaced several weeks apart. For the majority of patients, completing such a regimen will successfully address their volume loss concerns.

Results commonly last up to 2 years or more, making it a longer-lasting option compared to many traditional fillers. Our general recommendation is to have an annual touch-up treatment to maintain consistent collagen production. Patients who wait until results fully fade (between 2 and 3 years) may need to repeat another full regimen of treatment. Additionally, patients can maintain or enhance their results with other nonsurgical treatments such as fillers, laser treatments, or microneedling.

Traditional fillers add immediate volume using a formula with hyaluronic acid as the core ingredient. In contrast, Sculptra works gradually by stimulating the body’s own collagen production via a PLLA formula. Both options have advantages, and the decision is largely based around individual patient preferences.

Sculptra is mixed with a numbing agent to help improve comfort during injection, and most patients tolerate the treatment well. Following treatment, discomfort is typically mild and resolves within a day or two.
